I would get her to TED to eliminate the possibility that there's a physical cause. If there isn't then .... ???
I believe the very best thing would be to confine her in a smaller space until she uses the box every time. I know, I know.. limited space, hard to confine somekitty, but it's supposed to be very effective.

Huge ((((((( hugs ))))))) for you sweetheart. Hang in there. This WILL get better.
As for anchors - have you tried canned goods? I've done that with newspapers for the dogs on occasion. They're heavy enough to deter most kitties but not too hard to move.
I am so praying for a solution for you that will end this stress forever!
