Looking for info on choosing a Cosmetic Dentist?

A piece of my filling fell out of my tooth a day ago while chewing a piece of candy. The filling is approximately 3-4 years old. It is actually a second filling on the same tooth. My dentist recommended a crown if the filling started to fall out again. The previous filling lasted about 15 years.

Why did the second filling breakdown so fast?

Is it possible to just replace the filling or should I opt for a crown?

Do you have any recommendations for a reputable dentist in Rochester NY?
