Looking for info on choosing a Cosmetic Dentist?

anyone have one? my dentist has suggested one for years, and while i agree with her, i haven't actually done anything about it until my last visit. I asked for the price and was taken back when they said $500.

I looked for info online and saw sites that send you your own molding kit and then they mail you the guard for MUCH less money.

anyone try these sites with success? is it really any difference than the dentist? after all, isn't all the dentist does is take an impression and mail it off to a lab?
