Save or Die!
Basically the article proposes that in order to make someone vulnerable to a 'save or die' effect, that they must be weakened by hit point loss. I think that this is a good mechanic and in many ways makes sense, but I had previously read a different article from 2008 at the Alexandrian blog that I think is a better solution that uses the existing 3rd edition ability damage:
I really like the idea of linking the effect to a particular abillity. In addition to relying less on mistress luck by potentially requiring multiple 'hits', it uses an existing mechanic, and the effects can linger, which I think will help maintain the impact and drama of a potentially deadly encounter.
