George is correct on the first one: I have seen some of these iconographer's manuals, most of them in Greek, but there was (is?) an Orthodox printing outfit located in Torrance, CA that produced some of these things in English back in the - I think - '80's and '90's. If memory serves me, Phil Tamoush was the founder/organizer of this printing house.
Regarding the second, he's also correct, but there needs to be a bit of emphasis on his sentence, "Icongraphers do have some leeway..." There is a famous icon of Christ from Byzantium where clearly one side of his face is different from the other, and the reason given is that the icon depicts both his divinity and his humanity. I think this is the icon on the cover of one of Pelikan's books. But this may be a very small example of a very small exception...
Then again, I am not an iconographer! Just a few random thoughts.
