As usual - the outcome followed a series of unfortunate decisions. But not necessarily unjustifiable decisions. Whenever we do any design, we can always add more margins of safety until we have no money to spend elsewhere where benefit/cost ratio is greater.
Last year there were 325 fatalities in fire in UK, but about 14000 prostate cancer deaths. Where should we allocate a spend of an estimated £600 million pounds? Sprinklers or hospitals? Of course we could always borrow even more money from China and do both.
