There still may be time for the other one to drop but IMO the longer it takes the less likely. If it doesn't drop then yes you have to have him neutered and this would involve the vet having to remove the one that hasn't dropped. The reason is that it has a higher likelihood of becomming cancerous because of the body heat. They will have to do surgery on him. I don't believe the surgery is very risky it more like a spay. The cost will be higher though.
