All situations are, of course, different - so I can only tell you about our experiences. We've flown to Turkey many times with Sophie (who has DILV and TGA) over the past 6 years, which has spanned 2 of her 3 ops (she's now 7).
We do have to pay an extra insurance charge, but this is normally only around £35, and we do need to fly with oxygen. This somewhat restricts our choice of airline, and adds a little more bureaucracy, but we usually use Thomas Cook who do not charge (though we have never needed it). I'm sure there are others that don't charge as well, but there are certainly ones that do, and others that are not licensed to carry "personal" oxygen, so some care is needed when choosing/booking.
