Hi there Kerry - glad it was of some use!
We found out about the oxygen by asking at her check-up at St Thomas's (or Guy's as it was then) though I'm sure the outreach team would be able to get your question to the appropriate consultant.
The other details may vary with airline/insurance company, but we also have to go through medical screening (a few questions on the phone) to decide on the level of extra premium payable and get the cover finalized, and the airline require a "fit to fly" letter which we normally get from our GP (that can cost too - do you spot a pattern? - but ours is usually kind enough to do it for free).
