A lot depends on where you are located and how far you are willing to travel to get the work done. Also, if you don't mind doing the wrenching yourself, there are plenty of mail order companies that would have what you need to get the job done.
If distance is not a factor, then Wagoner Machine Shop in Haskell (918-341-1742 or 918-482-2669) builds trucks from mild to extreme.
Also, Sam's Off Road in Tulsa does a great job building rigs (918-446-5535). And there is 4Wheel Parts Wholesalers in Oklahoma City (don't know the number).
