It's likely the /l/ and /r/ problems you are noticing are totally unrelated to the tongue thrust the dentist mentioned. Tongue thrust doesn't necessarily cause speech problems. Many people have tongue thrusts that do not have any speech problems. A tongue thrust is a problem to the teeth however, because thrusters push their tongue aginst their teeth somewhere when swallowing and sometimes when speaking.
