open-mouth posture

I'm seeing a 15-year old boy who generally has an open-mouth posture at rest, with very poor articulation. He has a history of ear infections but not recently as far as I know. But he can keep his lips closed for a minute or longer, so this doesn't sound like low tone, or is it?
