Maybe blowing would be a next step? You could have him practice blowing with straws, bubbles, etc. After he masters blowing, you could have him do kind of a plosive blow -- where he makes a "p" sound first. Then on to kind of a raspberries type blow... then try it with a mouthful of water. (Sounds like fun, actually!) I wouldn't worry about getting the toothpaste out of his mouth at first -- until he masters the mechanics of spitting. The rest should come naturally if he gets used to doing it every time he brushes his teeth.
