I don't know about 20, but it *is* possible to get a 4-cigarette holder - I've seen Andi (SmokeScreen), Denise (Smoking Angel), Octavia (Smoke4Me) and Yasmine (Nicotine Delight) all using one. However they are rarer than hen's teeth to find on eBay or elsewhere - good luck! I know Andi got hers custom-made by a jeweller or tobacconist. The others were a sideline Yasmine was making, but I think she's left the fetish scene.
The easiest way to multi-smoke that I have seen described is the '6+1'. Get a single cigarette and put 6 around it in a hexagonal fashion (like the chambers of a revolver). For added stability you can use a rubber band to keep the cigarettes together. I've read of this method on a few boards.
