I don't know of any evergreen in that size range, maybe a miniature conifer would be. We do have some dwarf Colorado spruce but they get larger than 3 x 3.
How about Andromeda polifolia? Its common name is bog rosemary. It is an extremely tough plants and will survive in a varity of conditions, it is evergreen with blue-green leaves and pink flowers it has a very small shrub habit and I think will work well for your situation!

I was wondering if you could identify this plant by description. I pass it everyday going to work, I've never seen anything like it. 5 x 5 approximately...round habit small upright medium to light green leaves, and blush white/pink tips...I can't tell if it's going to flower into something or if the tips just stay that color. Don't know how I haven't noticed it before this year. Leaves are matte and look like they are soft in texture.
I'm sure that I could identify it if you could send or post a photo of it. Most likely its Salix 'Hakura Nishiki' based on your description but I'm not sure.
there are some junipers that we have at the nursery ,perhaps they might fit the bill. i would choose a nandina. there are compact varieties that remain short. how about an acuba. not blue but nice. L
