The connector screws directly to the antenna, the coax slips in a slot or hole in the mast at the top were boom mounts and runs inside the mast and out the bottom or close to it or some have the coax exit higher and you can see your coax twist around mast - then unwind.
You do not necessarily spin the antenna in a 360 more than once in the same direction and you natural unwind your arm if only one turn.
That is the typical setup and others have tried and still use other methods of bringing the coax in. This will also depend if you are going to transmit through it or not.
