Built from Trident/Rocket 3 parts "'cos we can". Apparently Dennis Poore was always moaning to Doug Hele that NVT didn't have a four-cylinder engine, so Doug and the NVT Engineering (pretty much Meriden Experimental as was before NVT) team built it in secret (to the extent of having Norman Hyde's dad machine the cams). Then, one day when Poore came in moaning as usual, Doug whipped the cloth off the strategically-placed engine and said, "You mean like this?".
The "offset to the right" is because the 'right middle' crankcase is a machined right-hand triple casting with another standard one outboard of that, the 'left middle' crankcase is a standard triple centre one, also containing the gearbox, that's placed as normal in a Rocket 3 frame so the gearbox sprocket lines up with a standard rear wheel sprocket.
