when I had a heart attack , they got to me within about 30 mins , however they were unable to "hand me over" at ther hospital and I was sat in back of an ambulanve for an hour and a half , there were also about 6 more ambulances sat waiting , THERE is your bottle neck ewhilst the mbulances were waiting to hand people over to the hospital , they could not go out on other jobs , ie if the actual hospital is busy , you do not get a paramedic and urgent care
