Here are some guidelines for Millennium Sunday teams on how to submit the Bollinger Insurance Player Waiver and Release Form.

All teams must fill out the Bollinger Insurance Player Waiver and Release Form and submit to the league no later than Sunday, May 3rd.

There are two ways to get the Bollinger Insurance Player Waiver and Release Form to the league.

One way is to contact your Millennium Sunday division captain and make arrangements on getting them the form.

The second way is to drop the form off on Sunday, May 3rd at Barnaby's of Ridley between 1:00 p.m. and 2:00 p.m.
