Here are some guidelines for Millennium Sunday Softball League teams on how to submit the NASF Insurance Player Waiver and Release Form.

All teams MUST fill out the NASF Insurance Player Waiver and Release Form and submit to the league no later than Monday, April 10th.

Note: The NASF Participant Release of Liability Waiver Form is to be used for players who weren't available to sign the team waiver form after it was submitted to the league.

To submit the NASF Insurance Player Waiver and Release Form to the league please contact your Millennium Sunday division captain and make arrangements on getting them the form.

Note: If any team(s) does not submit the waiver form by the above date your team and sponsor will be in jeopardy of not having any liability insurance.
