Down load the Trilingual health inspection from www.inspection.gc.ca.
Then I make an appointment with my vet for within 15 days before flying out.
I phone the local office of the Canadian Food inspection and make an appointment with their vet for after the appointment with my local vet and before flying out. (You may have to jiggle this a bit depending on the availability of their vet - but remember this all has to happen within 15 days of takeoff.)
At the appointment with my local vet the form is completed stating the date that my pet received the rabies shot as well as internal and external medication for ticks and fleas, together with Lot # of the medication, etc. Any other medication is also recorded at the same time.
This form is then signed, and stamped with the vets office stamp if available, together with the Vets license number.
At the appointment with the Canadian Food Inspection office I give them the form, which is later returned with certification from them that my pet is healthy.
I now have 2 forms to provide to the officers at the Mexican airport.
