I got my card in La Huerta several years ago. I believe I had to provide the usual - copies of ID, CFE bill.
I haven't had much luck using it at the local pharmacies, but get 1/2 price tickets on bus.
btw Guadalajara farmacia has deeply discounted prices on some medicines.
