The first person to register KathyB will get that username. The next person will get an error message saying that username is already taken and they can choose something different, but they should still make it recognizable for others.

KathyB2, KathyBoss, KathyBo or KBoss would all be options. Usernames that don’t give other people a good idea who you are would be changed or deleted by the moderator. So something like KB or Boss would probably be too nonspecific.
