How to determine ld?

My son is almost 9. He is extremely well spoken with a large vocabulary. He reads & spells very well. His problems are with handwriting & math. Writing is very difficult for him, beginning with holding the pencil correctly, to neatness, to being able to get his thoughts on paper. His math skills are unpredictable. Many problems he can answer verbally, but then cannot do them on paper. Memorizing the tables is near to impossible. What he seems to know one day is gone the next. He is becoming very frustrated. He has been diagnosed by an occupational therapist as having some fine motor skills issues. We have been trying for two years to work with the school. We have done, and are doing, extra tutoring and extra help at home. He just desperately wants better grades, now that they get letter grades, and despite his extra work, while improving, is now at a C level in math & writing, where his others grades are A's. What else can we do? Any suggestions as to how to approach the school are welcome. We have finally gotten them to assist with math help, but that is it, as he does well in the other subjects they do not see that he needs any additional assistance. Thank you!
