Whether using the new online apps or the downloaded/installed software, on the main screen you will find two yellow buttons, one for viewing/printing paper forms, and the other entitled "Online Entry Option". Clicking that button will display and then take you to the URL for the special online entry page. Provide that address to parents/teachers asking them to completed the form for the student being evaluated. Be sure to tell them to enter your email address into the designated field on the form so that the contents of the completed form will be emailed directly to you.
