A "bolt" has a hexagon at one end, and a "stud" is threaded at both ends.

If the inner nuts are the correct 12-point type then they don't require washers.

With great difficulty-unless you want to go hunting around for torque wrench offset extensions to fit the nuts. Simply do them up tight.
Or, if you want to get scientific, then you could use a lever and spring balance to torque up the nuts in which case you would need to work out the "pull" poundage required for the particular length of lever/spanner.
