The problem with this is that 'insane' is a legal term, not a medical one.

Psychiatrically, we are talking about 'rule by the personality-disordered'. Sadism, psychopathy and narcissism together make 'malignant narcissism': I have recently seen Trump described by a professor of psychology as 'the most perefct example I have ever set eyes on'. Same guy diagnoses narcissistic personality disorder in Obama too. The 'mental health challenges' of such individuals are as nothing compared to the damage they do when they take charge of something or someone. At the very least, they need to be filtered out of the political process.
