I sensed from the start it had to be something like this, knowing of the consistent impeccable high moral standards followed by the Defence Force: the most moral army in the world you know?
And did you know there was "poor night visibility" caused by the Earth having rotated away from the Sun?
That "operational misunderstanding" can happen to the very best of us. If combined with an over keen young deputy committing a bit of light-hearted "breach of orders" any old mis-hap can occur. And with planetary rotation throwing in unexpected "poor night vision" - goodness knows.

The BiBiC 'news' article smacks of being through a gazillion editors to get the 'honest and open' tone 'sincerely examining a sad event' just leaning on the side of the 'mistakes happen' line.
