Sorry to hear of your troubles with this Gerard. As ever, it seems care is different depending on where you live. My own GP has also introduced an online form, as they all have to now, but they say on their website that if you don't have internet access or would prefer to speak to someone then you can call and they'll be happy to fill in the form for you. Doesn't get round the employment question of course, although the one time I used mine I don't remember a similar question - I may be forgetting though. I hope they address your complaint properly.
