Now, the 'attempted' in the title has the effect of making whatever the Iranians did seem a bit shit. So where does it come from?

It comes from the body of the article:

"The US military says it has completed a "heavy wave" of strikes against Iran in retaliation for Tuesday's attempted ballistic missile attacks on American forces."

So, the failure-word 'attempted' actually originates in A STATEMENT FROM THE US MILITARY, FFS...

There is hardly a single thing I find the BBC putting out nowadays that isn't bent...
