Ignoring the hoax/satire about Pfizer CEO being arrested, the main issue about these figures seems to be that the number of total vaccines administered hasn't been released, so we can't tell what percentage the 42K adverse events and 1,223 fatalities are of the total for that 3 month period. Oddly this seems to have been redacted from the released document: '3.1.1. I searched the doc to see what this '(b) (4)' designation meant but there wasn't a key or other explanation anywhere, just another couple of apparent redactions about info relating to Pfizer's employees and resources available to them.
Looking at a few 'fact-check' pages on this - most of them from corporate media or affiliated to them, it must be admitted - they make the point that adverse effects and fatalities aren't necessarily related to the vaccine but could be pre-existing or unrelated conditions.It also depends on VAERS-type voluntary reporting of symptoms which are not verified as related to the vaccine (though there also exists the possibility of under-reporting). Eg this from newsweek which puts the total number in the 'billions':

42K adverse reactions and 1,223 fatalities out of 'billions' of doses would indeed be a small percentage and support the document's conclusion of a 'favorable benefit: risk balance for [the Pfizer vaccine]' (p.29 ibid.) even if it wouldn't support reassuring or self-serving claims of 100% safety & efficacy being made by politicians & big pharma execs at the time. Unfortunately we only have Newsweek's word for it until Pfizer see fit to confirm the total amount of doses they've drawn this data from.
