Notifying DVLA depends upon how diabetes is controlled. If is controlled ONLY by diet you do not have to notify.

Control by insulin injections and certain tablet medication requires notification. I guess that the need for a paper application form is so that you can confirm your dosage of tablets and to check that the treatment has not changed from the last notification. Seems perfectly reasonable.
