It really isn't unusual for a 5-year-old to occasionally write in mirror image. This usually ends in first grade but sometimes will continue slightly longer without actually indicating a significant problem. However, if by second grade he is also showing more generalized delays in reading and/or writing an evaluation for possible special education needs might be considered.
