Hydraulic tappets are an issue so i believe.
However if you are seeing injector pulses 'on' and loss of CTS...Monitor the powers and grounds of the ecu when fault on ie any volt drop which could cause this scenario.

Check the injector loom is not chafed through in the loom around or under inlet manifold,i have seen this.Open it up fully to eliminate it.
I would also scope your primary patterns for the coil fault when fault happening.

If you stand back from it there are many errors and goings on that may have a common cause!
Try to decide mechanical or electrical or both!

If this is happening warm as you say i would scrutinize the engine man side personally
