You haven't posted any emission readings at normal or fast idle which might give a clue. Is the Lambda sensor stuck at lean across the whole rev range? Also, by disconnecting the MAF I would expect to see a richer mixture which in your case appears to compensate for the lean mixture. Finally, how did you check for air-leaks?
