this means he has such strong conviction that he considers it unethical to undertake a randomised blinded controlled trial then the next best thing would be to do a propensity score matched study in which he compares the outcome in cohorts of matched patients from other institutions not using his treatment. As repeated at length - the large majority of people recover without treatment so the characteristics of the cohort treated has a large impact on the outcome. If I give HSQ and AZ to 1000 20-29 yo with mild infection and none die then it's the group I chose not the drug which has had the biggest effect. From recollection Azoulay did not publish the demographics in his 1000 person study. I'm not persuaded by his arguments at all. He could have published very robust studies many times over. If he has treated 15000 people the lack of basic science is shocking but keeps him in the headlines.
