All my cats have been strays/ferals. When you think about it, they managed to survive out in the world and alone. I figured if I left enough dry food (although my cats have always eaten 90% canned) they'd survive two days without coddling.
For a cat who pees outside the box and doesn't have any medical problems, I would think she's stressed and would try to add a box so she feels more comfortable using it.
