    hi John don't give up, we all get these now and again,iam fearful of suggesting something you my have all ready tried but not mentioned in your posts so don't shout,
    check for air leaks in front of o2 sensor i.e.
    block exhaust spray head to manifold manifold to down pipe,around o2 sensor ect
    does fuel pressure follow vacuum correctly while on the subject your vac figers seem high at fast idle ?. give me engine code may be able to give you some figers to compare with.regard nige.
